Associate Patient Care Coordinator at Optum summary:

The Associate Patient Care Coordinator supports primary care teams by managing patient scheduling, processing referrals and prior authorizations, and maintaining accurate patient records using electronic health record systems. They ensure smooth office operations, provide excellent customer service, and assist with outreach to patients to improve appointment compliance and care continuity. This role contributes to achieving financial, clinical, and service goals while upholding patient confidentiality and healthcare standards.

We have an immediate opening for a friendly, patient focused and detailed oriented Associate Patient Care Coordinator (Care Team Support) to join our team. The Associate Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service, and adhere to Lean processes. Supports the teams in meeting financial, clinical, and service goals.

Location: 5 Neponset St. Worcester, MA ( Primary Care Department - Third Floor )

Schedule (38.75 Hours) : Monday-Friday, 8am-4:30pm and 8am-5pm EST. Schedule will vary depending on business need.

Primary Responsibilities:

• Enter orders and referrals, and sort faxes as requested by primary care provider or care team
• Process prior authorizations
• Collect or verify demographic information, including key demographic fields
• Load or update insurance information as needed
• Verify eligibility and flag appointment accordingly
• Schedule appointments using approved scheduling guidelines
• Use call processing and other applicable functionality in the EHR to take messages and communicate with clinicians
• Monitor daily reminder call logs; conduct outreach to patients requiring appointments; contact patients who, "no show," for follow up appointments for chronic conditions or preventive care
• Monitor administrative tasks in the EHR inbasket and respond timely
• Scan and index documents into the EHR; ensure that necessary documents, lab results, etc. are available in advance of patient visits
• Assist coworkers as needed to assure smooth office operation and delivery of excellent service through teamwork
• Demonstrate respect for patients and staff by presenting a friendly, courteous manner at all times and maintaining patient confidentiality
• Perform other duties as assigned

Required Qualifications:

• High School Diploma/GED (or higher)
• 2+ years of customer service experience
• 1+ years of receptionist experience
• Access to reliable transportation and willing and able to travel 10% of the time to cover other practices, if needed

Preferred Qualifications:

• 1+ years of experience as a receptionist in a healthcare setting or 5+ years of customer service experience
• Experience working with an electronic health record (EPIC)
• Intermediate level of computer proficiency (including Microsoft Outlook and Teams)

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $16.00 to $27.69 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
