An Acute Care Medical Provider is needed to help us in our endeavor to build the healthiest community in the nation … one person at a time.

This position will be based at the new and innovative acute care behavioral health facility, which opened in late 2023.  SummitStone has the privilege of partnering closely with Larimer County on this venture.  This voter-approved facility provides licensed, equitable and culturally relevant care and treatment to individuals in crisis with mental health and/or substance use disorders.  For more information click HERE.

What You’ll Do:

As an Advanced Practice Provider - Physician Assistant or Nurse Practitioner, the employee will provide direct professional services in the evaluation and treatment of clients at our new, state of the art, Acute Care facility at Longview campus. You may see clients in the behavioral health urgent care and/or the inpatient units (crisis stabilization and withdrawal management). This position will provide a comprehensive approach to client care focusing on behavioral health, substance use and chronic physical health disease management. Major responsibilities of this position include: function as the medical team representative to a multidisciplinary treatment team designed to offer comprehensive behavioral health services; evaluate patients for behavioral health, substance use and medical disorders, determine the appropriate course of treatment for the patients; ability to thrive in a multidisciplinary team setting; meet/maintain documentation standards; and comply with county, state and federal laws, rules and regulations pertaining to health care.

This position requires knowledge of diversity and inclusion with the relevant ability to engage, communicate, interact, and work effectively and cooperatively with people of different backgrounds, identities, and culturally diverse communities.

Required :

• Advanced Practice Nursing degree (APN) degree or Physician Assistant (PA) from an accredited university.
• Demonstrated commitment and experience working with persons from marginalized communities and other patient populations experiencing vulnerabilities.
• Understanding and commitment to Medications for Addiction Treatment and harm reduction principles.

Preferred:

• Spanish language skills. Other language skills also beneficial.
• At least 2 years' experience treating/servicing persons with substance use and mental health needs.
• Strong understanding of substance use treatment and knowledge of withdrawal management processes.

Some positions include driving a company vehicle and require a valid Colorado driver’s license.

Total Rewards

The typical hiring range for this position is 113,506 to 141,887/yr based on full-time work and based on relevant years of experience.
